Sign-Up - Fidelis Care secure Provider Portal

Thank you for your interest in obtaining access to Fidelis Care’s secure Provider Portal. By submitting the information below, you are starting Step 1 of a 2-step process to gain access to your organization’s data on our secure Provider Portal. In order to only provide the right information to the right organization, we need to gather certain background information so that you and your organization are profiled within our systems.

By clicking on the appropriate Sign-Up option below, you begin the following Step 1 process your request will go through:

• Request for Service – You will enter all needed data and you will receive acknowledgement from us via email of the receipt of your request.
• Verification – We will take various steps to confirm your need for access. We will only contact you if we have questions.
• Build Profiling – We will profile your organization within our secure Provider Portal system and contact you if further information is needed. Once this process is completed, we will send an email with your login instructions.

Once you receive your login instructions, you must follow the Step 2 instructions we will provide you to allow actual login.
