People with complicated medication therapy can be helped through the SilverScript Medication Therapy Management (MTM) Program. This program is not a Medicare benefit, but is being offered as an additional service to help you and it is available at no extra cost to you.

 

The program works with you and your doctor to make sure you are receiving safe and effective medication therapy. Specially trained pharmacists will review your medications. They will discuss with your doctor ways to avoid possible adverse events or ways to help you better manage your medications.

 

In addition, you will receive newsletters four times a year which contain information on preventing and treating common health-related issues. These newsletters will include information on new drugs, tips for staying healthy and other information to help you get the most out of your medication therapy.

 

If you take more than five medications every day, have two or more long-term health conditions (e.g., diabetes, asthma or high blood pressure) and spend more than $4,000 a year on medications we will automatically enroll you in this program. If you do not wish to take part in the program, call Customer Care at 888-831-3048 and ask us to remove you from the Medication Therapy Management Program.
