Fidelis Care Child Health Plus, a New York State-sponsored health insurance program, offers comprehensive health insurance coverage to children who live in New York State up to their 19th birthday. Regular health checkups; immunizations; routine dental and eye care; prescription medications; hospital inpatient, outpatient, and emergency care; surgery; and much more are completely covered, with no cost to the insured member. Members may pay a premium monthly for the coverage depending upon their family earnings and family size.

Child Health Plus has a year-long open enrollment period. To meet with a Fidelis Care Representative, call 1-888-FIDELIS (1-888-343-3547) or email us. We'll meet with you in your home, at one of our local offices, or at a convenient location in the community near you.
By entering a zip code, date of birth, number of family members, and household income, the Fidelis Affordable Care Advisor can let you know what you can expect to pay. It also allows you to schedule an appointment with a representative who can assist with enrollment or apply directly through NY State of Health.

Child Health Plus Preferred Drug List (Formulary) (.pdf) | A drug formulary is a list of both generic and brand name prescription drugs that are covered by your prescription drug benefit. The formulary is managed by our Pharmacy and Therapeutics Committee, qualified pharmacists and doctors whose primary focus is offering safe, high-quality and cost-effective drugs. New drugs are added to the formulary periodically based on the latest medical research and some drugs may be removed periodically based on the findings and recommendations of the Pharmacy and Therapeutics Committee.
