Fidelis Care Child Health Plus, a New York State-sponsored health insurance program, offers comprehensive health insurance coverage to children who live in New York State up to their 19th birthday. Regular health checkups; immunizations; routine dental and eye care; prescription medications; hospital inpatient, outpatient, and emergency care; surgery; and much more are completely covered, with no cost to the insured member. Members may pay a premium monthly for the coverage depending upon their family earnings and family size.
Fidelis Care’s Child Heath Plus program was ranked first in the State of New York for quality of care and service in New York State’s Department of Health 2011 and 2012 Consumer Guide.

Every child under 19 years of age in New York State is eligible for Child Health Plus.
Depending on how much you earn and the size of your family, coverage may cost very little or, in some cases, nothing at all. The chart below illustrates monthly premiums per child by level of monthly income (your gross, pre-tax household monthly income). These premiums periodically change.
