Fidelis Care Child Health Plus, a New York State-sponsored health insurance program, offers comprehensive health insurance coverage to children who live in New York State up to their 19th birthday. Regular health checkups; immunizations; routine dental and eye care; prescription medications; hospital inpatient, outpatient, and emergency care; surgery; and much more are completely covered, with no cost to the insured member. Members may pay a premium monthly for the coverage depending upon their family earnings and family size. Fidelis Care’s Child Heath Plus program was ranked first in the State of New York for quality of care and service in New York State’s Department of Health 2011, 2012, and 2013 Consumer Guide.

| Fidelis Affordable Care Advisor
The Fidelis Affordable Care Advisor is an online calculator that can help you plan for and estimate the costs of new insurance options, including Child Health Plus. By entering a zip code, date of birth, number of family members, and household income, the Fidelis Affordable Care Advisor presents insurance options at prices that include subsidies or tax credits, if they apply. It also allows you to schedule an appointment with a representative who can assist with enrollment.

The formulary is managed by our Pharmacy and Therapeutics Committee, qualified pharmacists and doctors whose primary focus is offering safe, high-quality and cost-effective drugs.
