Health Equity

At Fidelis Care, we believe everyone should have the opportunity to live a healthy life. It’s our mission to ensure New Yorkers have access to high-quality healthcare, so they can get the care they need when they need it.

But many other factors contribute to an individual’s health and wellness beyond access to healthcare. Socioeconomic conditions can influence health risks and outcomes. Poverty, food insecurity, housing instability, education, employment, access to transportation and other circumstances contribute to health disparities among underserved and vulnerable populations.

Fidelis Care is committed to removing those barriers to health to improve access, quality, and affordability. It is an ongoing process that requires working together with our members, providers, and community-based organizations to support fair and just opportunities to equal access to healthcare.

Key partnerships

Wellness commitment to Buffalo Urban League

To nurture social entrepreneurship, facilitate wellness, and strengthen organizations focused on Black, Indigenous and People of Color in Buffalo’s East Side, Fidelis Care and the Centene Foundation donated $1.1 million to the Buffalo Urban League (BUL) to help establish its new headquarters and develop a Wellness and Entrepreneurial Center.

In addition, Fidelis Care partners with BUL through community programs and at events held in the city focused on health, family support and stabilization services, foster care, adoption, education, job training, employment, scholarships, and more.

Mental health alliance with The Jed Foundation

As part of our behavioral health efforts, Fidelis Care and the Centene Foundation awarded $1.1 million to The Jed Foundation (JED) to protect the mental health of New York State’s youth.

Through the funding, JED will expand its current services, providing at least five youth-serving community-based organizations (CBOs) with consultation or strategic planning services, including expert guidance, educational workshops, and training programs, equipping young people with life skills and connecting them to mental healthcare when they are in distress.

Fidelis Care Plan Updates for 2026

Fidelis Care would like to inform our providers of plan changes that will be effective January 1, 2026. 

 

Wellcare By Fidelis Care

Wellcare Fidelis Dual Plus (HMO D-SNP) – Plan 003

Effective January 1, 2026, the plan name will change from Wellcare Fidelis Dual Plus (HMO D-SNP) to Wellcare Fidelis Dual Align (HMO D-SNP).

Wellcare Fidelis Dual Access (HMO D-SNP) Plan 001

Effective January 1, 2026, Fidelis Care members currently enrolled in this plan will be enrolled in Wellcare Fidelis Dual Liberty Sync (HMO D-SNP).

 

There are two segments for this plan. The service area for Plan 013-001 will be Albany, Allegany, Broome, Cattaraugus, Cayuga, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Franklin, Fulton, Genesee, Greene, Hamilton, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Ontario, Orange, Orleans, Oswego, Otsego, Putnam, Rensselaer, Saratoga, Schenectady, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Sullivan, Tioga, Ulster, Warren, Washington, Wayne, Wyoming, Yates.

 

The service area for Plan 013-002 will be Bronx, Kings, Nassau, New York, Queens, Richmond, Rockland, Suffolk, Westchester.

Wellcare Fidelis Patriot Simple (HMO-POS) Plan 010

Effective January 1, 2026 Fidelis Care will offer a new Medicare Advantage product named Wellcare Fidelis Patriot Simple (HMO-POS).

 

The service area for this plan will be Albany, Allegany, Bronx, Broome, Cattaraugus, Cayuga, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Franklin, Fulton, Genesee, Greene, Hamilton, Herkimer, Jefferson, Kings, Lewis, Livingston, Madison, Monroe, Montgomery, Nassau, New York, Niagara, Oneida, Onondaga, Ontario, Orange, Orleans, Oswego, Otsego, Putnam, Queens, Rensselaer, Richmond, Rockland, Saratoga, Schenectady, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Suffolk, Sullivan, Tioga, Ulster, Warren, Washington, Wayne, Westchester, Wyoming, Yates.

 

 

 

Ambetter From Fidelis Care (Metal-Level Products)

Wellness Rewards

Effective January 1, 2026, members can participate in the Wellness Rewards program where they can earn up to $325 by completing eligible activities and screenings. Eligible activities are worth $25 each and include an Adult Well Visit, Adult Immunization Flu Shot, Breast Cancer Screening, Cervical Cancer Screening, Childhood & Adolescent Immunizations, Chlamydia Screening, Controlling High Blood Pressure/Blood Pressure Screening, Colorectal Cancer Screening, Diabetes – HbA1c Control, Diabetes – Eye Exam, Follow-up After Acute Care Visit for Asthma, Kidney Health Evaluation, Follow-up Visit After Hospitalization.

Ambetter from Fidelis Care Silver Enhanced

Effective January 1, 2026, the plan name will change from Ambetter from Fidelis Care Silver Enhanced to Ambetter from Fidelis Care Silver+.

 

 

Essential Plans

Wellness Rewards

As EP members renew into 2026 coverage, they will become eligible for the Wellness Rewards program where they can earn up to $275 by completing eligible activities and screenings.
