The following sections of the Fidelis Care authorization grids have been updated effective September 1, 2021.
The following services apply to Medicaid, Essential Plan, & Metal Level Plans and require prior authorization:

The following code change will apply to Medicare Plans and requires prior authorization:
J1561 immune glob, now “Gamunex-C” - was previously "Gammagard"
