Fidelis Care provides voluntary Population Health Management (PHM) programs that meet the care needs of its entire member population, across the continuum of care. The PHM programs focus on the “whole person” and the member’s entire care journey.
We provide important educational information to members in our newsletters, on our website, fideliscare.org, and through customized letters, brochures, and other mailed materials. Members who do not require active case management may be supported by other outreach initiatives. Fidelis staff monitors for gaps in care, and conducts follow up calls with the member’s provider, as well as provides health and wellness information.
Fidelis Care offers the following PHM programs and services:
- Keeping members healthy with flu shot reminders
- Complex case management programs for members with hemophilia and sickle cell disease
- Transitional care case management program after a hospital discharge
- Referrals to Health Homes
- Care coordination services for members with behavioral and medical needs
- Medication reconciliation
Fidelis Care regularly evaluates the effectiveness of our Population Health Management programs, and values provider feedback and participation in this process.
