Renewal and Recertification Information

Every year, members enrolled in Fidelis Care's Medicaid Managed Care, Child Health Plus, the Essential Plan and Metal-Level Products must renew their coverage. This process for renewing your coverage differs depending upon how you initially enrolled.

If you are a Healthier Life (HARP) member, you will still renew coverage through your local LDSS or HRA. 

Members that enrolled through a Department of Social Services (LDSS) or the Human Resources Administration (HRA):

You will receive a written notification 30-90 days before your renewal date. Be sure to review the notice carefully and follow the instructions. Submit the required forms and documentation directly to your LDSS or HRA.

If your child has been continuously enrolled with Fidelis Care's Child Health Plus product prior to the creation of the NY State of Health Marketplace in 2013, Fidelis Care will mail you a renewal form 90 days before your child's renewal date. Complete the renewal form, sign, date and mail as soon as possible in the envelope provided. Include any required proofs if necessary. 

Members that Enrolled through the NY State of Health:

You will receive an email and/or a written notification from the State of New York 60-90 days before your renewal date. Be sure to read and follow the instructions provided in the letter. To maintain your coverage, you may be required to update your account information on the NY State of Health website. 

A New Way to Renew Your Coverage
Did you recently receive a letter that looks like the one as shown? Starting this year, instead of renewing your coverage by filling out a paper form or visiting your local Department of Social Services, you will now renew your coverage by visiting the NY State of Health website. 

Confused about your next steps and what this means? We can help--it's as easy as 1, 2, 3.

1. Visit and click on "Get Started." Follow the instructions to create a NY.Gov ID if you don't already have one.

2. After logging in, you will need to enter your invitation code. This is included in the letter you have received (located under Step 2 in the instructions).

3. Follow the instructions on the NY State of Health website to finish your renewal.
