IMPORTANT REMINDER! New Claims Correspondence Post Office Boxes

As a reminder, effective immediately, the provider claims correspondence listed below* must be mailed to the new Post Office (P.O.) Boxes according to the member’s product as indicated:

 

 

 

*Excludes: New claims, corrected claims (These should continue to be submitted as indicated in our provider manual).

Please note, during the initial transition period, mail will be accepted if sent to the wrong address.  However, after the initial transition period, mail sent to the incorrect address may be returned to sender. 

Be sure to share this new information with your office staff and billing staff as necessary.
