Authorization Changes Effective October 15, 2021 – DME, GI, and Sleep Study

Effective October 15, 2021, Fidelis Care will be moving a number of authorization requests to our portal and our fax line. Services related to Durable Medical Equipment (DME), GI related authorizations and Sleep Study requests for services must be submitted to our fax line at 1-716-200-1389 or submitted through the Fidelis Care Portal. It is important to note these services will no longer be authorized via phone unless considered an urgent clinical request.
Log in to Provider Access Online (The Provider Portal) at your own convenience. The system can be accessed at any time, 24 hours a day, 7 days a week.
If you do not have a Provider Access Online user account established, and if you need assistance with setting up an account, please contact the Provider Call Center at 1-888-FIDELIS (1-888-343-3547) and a representative will be glad to assist you.
