Provider Satisfaction Survey

For the 19th consecutive year, Fidelis Care wants to know more about your experience with us. Your office may receive a survey in the mail requesting you to complete a survey, which may be completed by paper or online.

Fidelis Care places a high degree of importance on provider satisfaction. As a health plan, we consider all of you to be our partners in the provision of quality care and service to members. The opinions of your practice are an important source of information that help us identify and deliver solutions that best meet your needs and that streamline our work together. As in the past, the results of the survey will be used to improve the level of service provided by Fidelis Care and our staff.

Although Fidelis Care is not required to administer this survey, we consider it to be a helpful tool and resource as we seek to ensure strong relationships with our providers.
