Wellcare Fidelis Dual Align is a Medicaid Advantage Plus and Dual Special Needs Plan for individuals who have, or qualify for, Dual Advantage and receive an eligible score on an assessment conducted by an independent nurse evaluator. Enrollment in the program includes coverage for all of your Medicare and most of your Medicaid services.

• Members are assigned a designated nurse care manager 
• Receive individual attention and care planning to help them remain in their own homes
