COVID-19 Vaccination Outreach Campaign

As your health care partner, Fidelis Care is reaching out to our members (your patients), and encouraging them to obtain the COVID vaccine if they have not already done so.  As a first step, members will be asked to participate in a survey and notify us of their vaccination status. Our message also encourages unvaccinated members to schedule an appointment with you, as their health care provider, to confirm that they are medically eligible to receive the vaccine, and to make arrangements to obtain it.  Members are also advised that they can receive information about the vaccine and where to schedule a vaccine appointment by visiting Vaccines.gov.

 

We appreciate your support and partnership as we work together to make sure every eligible member is vaccinated.

Starting January 1, 2025, members in our Medicaid Managed Care, HealthierLife (HARP), Fidelis Care at Home, and Wellcare Fidelis Dual Plus plans will have access to a network of service providers to identify and address needs such as housing, food, transportation, and more.
