Fidelis Care would like to remind our providers to kindly submit your Fidelis Care Medicaid Chart Review request. Fidelis is required by the Department of Health and Human Services (HHS) to submit complete diagnostic data for Medicaid members for Medicaid Risk Adjustment. To help us obtain the necessary medical records for submission, we have partnered with Optum and CiOX Health.
You should have received a list of Fidelis Care Medicaid members whose records we need for service dates starting January 1, 2020, to December 31, 2020. We kindly ask that you provide these records by July 21, 2021. If you do not have a list of these members, please contact the number below.
Records can be submitted by fax, mail or via the CiOX Portal.
