Get Started

Take charge of your child's health! Get the care and services they need to stay as healthy as possible.
Step 1: Sign Up for Our Member Portal

The Member Portal is the easiest way to get information about your child's plan. You will have 24/7 access to your child's account. You can:

  • View and print new ID cards
  • Check your child's claims
  • Access your child's plan documents
  • Make a premium payment

    Step 2: Fill Out Your Child's Health Risk Assessment

    The Health Risk Assessment is a brief survey. It helps us understand your child's healthcare needs and gets them connected to services. Complete the survey that came in your child's welcome materials. Then mail it to us in the postage-paid envelope.

    Step 3: Get to Know Your Child's Benefits

    Fidelis Care's Child Health Plus plan offers a wide range of services. They include:

  • No-cost annual checkups
  • Immunizations
  • Preventive care
  • Routine screenings
  • Hospital and emergency care
  • Eye exams
  • Dental care

  • You can learn more about your child's benefits by reading their Subscriber Contract. To request a paper copy, call Fidelis Care at 1-888-FIDELIS (1-888-343-3547); TTY: 711.

    Step 4: Choose a Primary Care Provider

    Fidelis Care wants to make sure your child gets the best care. A primary care provider (PCP) helps keep your child healthy. If your child already has a PCP who is in the Fidelis Care network, then that provider should have been automatically assigned as their PCP. You can view or change your child's PCP from their Member Portal account.

    Find PCPs near you by using our Find a Doctor tool. Once you find a PCP, please call Member Services at 1-888-FIDELIS (1-888-343-3547); TTY: 711, and a Representative will update your child's account.

    Your child's PCP helps them stay healthy by providing, helping to arrange, and supporting authorization for medical services.

      Step 5: Schedule Your Child's Annual Checkup

      It’s important your child gets a checkup at least every year to see if there are any changes to their health. Your child's PCP can help them stay healthy and keep down your child's medical costs. Call today to make an appointment. There is no copay for your child's annual checkup.

      Here are some questions you might ask your child's PCP:

    • Does my child need any vaccinations?
    • Does my child need any health screenings or blood tests?
    • Does my child need any changes to their prescriptions?
    • What lifestyle changes can we make to improve my child's health?
    • When should I schedule my child's next visit?

    • Follow your child's PCP’s advice. Listen to your child's PCP and ask questions if you don't understand. Be sure to keep all follow-up visits. Also, make sure your child takes medications as prescribed.

      Plan Basics

      There are no copays for covered services, including preventive services. Always check with your child's primary care provider to find out if a referral is required for any additional health services they might need.

      Your child's coverage also helps:

    • Get high quality care. Access to high quality in-network care for little to no out of pocket costs.
    • Stay healthy. Health insurance helps your child get preventive care that keeps them healthy and in touch with a doctor.
    • Prepare for the unexpected. If your child has an emergency or becomes seriously ill, health insurance helps give you peace of mind knowing your child's healthcare needs will be met.

      Monthly Premiums

      If your child has a monthly premium payment, you will receive an invoice in the mail. You may also receive an email to pay your child's bill. Please make your payment by the due date indicated on your child's invoice to avoid possible loss of coverage. Refer to these Frequently Asked Questions (FAQs) for more information.

      Get Your Immunizations

      Fidelis Care encourages you to get your child the flu vaccine every year and any other recommended vaccines. These vaccines are free as part of your child's Child Health Plus coverage.       Sports Physicals vs. Annual Well Visits

      Students who play sports often need to have a sports physical, which are sometimes provided through the school. A sports physical does not replace an annual checkup with a child's doctor. During an annual checkup, or well visit, the child has a complete physical exam. The doctor also makes sure the child has the right immunizations and answers questions. Annual well visits are a covered benefit, and children should have one each year.

      Important Information on Coverage Decisions

      Each day, Fidelis Care's Utilization Management (UM) Department makes decisions on many health insurance claims. These decisions are based only on appropriateness of care and the existence of coverage. 

      Fidelis Care does not reward providers or other individuals for issuing denials of coverage, and does not offer financial incentives to UM staff based on decisions that promote underutilization of services. 

      Fidelis Care is committed to ensuring that members have the care and services they need.
