Fidelis Care Provider Pulse Survey

As a part of our continuous quality improvement efforts, the Fidelis Care Provider Relations Department is seeking your feedback through a short survey that will be delivered via email. The survey will help us identify and deliver solutions that best meet your needs and streamline our work together.

Surveys will be sent out in the beginning of each month to providers that have met with their Provider Relations Specialist the previous month.  The survey link will be sent from SMProviderRelationsFeedback@fideliscare.org, and will only take a few minutes to complete.
