Requesting NYS Health Insurance ID Cards

To request Member ID cards for your Fidelis Care NY Health Insurance plan, you must first visit our member portal.

To do so, visit Members.Fideliscare.org.


Below, we provide a step-by-step instruction on how to request, replace and/or download a temporary Member ID card with your NY health insurance information.

 

Accessing the Member Portal on the Fidelis Homepage

If you're having issues finding the Member Portal on the SERPs, you can easily access the Member Online Portal on the Fidelis Care homepage.

Click "Login" on the top-right corner of your screen, and select "Member Online Portal."

Users can also find this link under the main navigation dropdown by hovering over the purple block at the top-left of your screen titled "Member."

Register your Account (or login) to Request a NY Member ID Card

If you have already created an account or had one established by a Fidelis Care representative, simply sign in to your account by entering your email address and password, and click "Login."

Otherwise, select the "Register" button and you will be prompted to create an account, enabling access to the Fidelis Care Member Portal.

 

View, Print or Reorder a Member ID Card

After logging in, scroll to the bottom-left of your screen to the card titled "ID Cards," and select the purple button titled "View ID Cards."

Print a Temporary Member ID Card

If you have an appointment within the next 7-10 business days, you may want to print a temporary ID card now.

To do so, simply check the text box (as seen in the screenshot to the left) before you make your final submission by clicking the turquoise "Submit ID Card Request" button.

 

How long is the delivery process for a new Member ID Card?

After submitting your ID card request, you should receive a pop-up screen that indicates a successful order.

For new and replacement ID cards, members should expect to find their new cards in the mail in 7-10 business days

If you have not received your ID card after this time, please call Fidelis Care Member Services at 1-888-343-3547.

Temporary Health Insurance Cards for Members

After submitting your ID card request, click anywhere on the screen to remove the pop-up banner.

Now, click the "View Temporary Cards" button and you will be taken to the screen below.

Print a Temporary Member ID Card

Once you arrive to the page, you should see your unique Member ID Card populated in the left-hand side of your screen.

This will include your Member ID#, Member Name, PCP Name, PCP Telephone, CIN#, and our direct line to Member Services.

If the information is accurate and up-to-date, click the "Print" button, and you will be able to use this temporary card at your appointments until your official card arrives in the mail.
