About Us

As a faith-based, mission-driven health plan, Fidelis Care provides quality, affordable health insurance coverage for children and adults of all ages and at all stages of life. 

Founded in 1993 as the Catholic Health Services Plan of Brooklyn and Queens, Fidelis Care began with a mission to serve the uninsured and medically underserved in two zip codes of Brooklyn. Nearly 25 years later, Fidelis Care has grown - both in the scope of its commitment to New Yorkers and the access to care and services that it provides. 

Today, through a Statewide network of nearly 70,000 providers, the Plan serves more than 1.6 million members in all 62 counties, making Fidelis Care one of the largest health plans of its kind in New York State. 

Fidelis Care provides health insurance coverage through Qualified Health Plans, Child Health Plus, Essential Plan, and Medicaid - available through the NY State of Health Marketplace - as well as through the Medicare Advantage, Dual Advantage, and Managed Long Term Care programs. 

Grassroots outreach - from inner-city neighborhoods to the most rural towns - is central to the mission of Fidelis Care. From provider offices, to schools, churches, food pantries, and community agencies, Fidelis Care works closely with hundreds of partners to reach local residents in need and to be a resource in the areas we serve.

As part of Fidelis Care's corporate commitment, the Plan has provided millions of dollars in charitable grants and contributions for health and community-based initiatives in local communities and provider-based locations. Counseling for at-risk students and families, reducing unnecessary emergency room visits, transportation assistance, and health education programs for immigrant communities are just some of the initiatives that have benefitted from Fidelis Care's support. 

Fidelis Care's regional offices are located in New York City, Albany, Syracuse, Rochester, and Buffalo, with satellite offices in Suffern and Poughkeepsie. Twenty community offices located throughout the State provide Fidelis Care with a strong local presence and the ability to serve local residents who are members or who have questions about health insurance for themselves or their families. 

From the beginning, Fidelis Care has worked to be part of the social fabric of local communities, impacting people's lives with one of the most basic human rights - access to quality, affordable health coverage and care.
