Nutrition plays a vital role in managing chronic conditions and supporting holistic care.  Through a partnership with Foodsmart, Fidelis Care now offers eligible Medicaid Managed Care members access to personalized tele-nutrition support from registered dietitians – fully covered by Fidelis Care.

Foodsmart’s registered dietitians can help your patients:

• Manage chronic conditions like diabetes, hypertension, and obesity through ongoing, personalized nutrition coaching.
• Create practical, healthy habits with culturally tailored meal-planning tools and thousands of budget-friendly recipes via the Foodsmart platform.
• Access Foodsmart Bucks to purchase fresh groceries delivered directly to their doorsteps.
• Connect to community resources, including food pantries, SNAP enrollment, and other nutrition-related support programs.

Clinical outcomes from working with Foodsmart:

• More than a third of individuals with obesity lost more than 5% of weight
• 22% of Foodsmart members with diabetes improved their HbA1c levels to normal
• 33% of users with stage 2 hypertension returned to stage 1 or lower
• 36% of Foodsmart members with dyslipidemia improved their lipid levels to normal
