Through the Fidelis Care grant, Ardent Solutions plans to establish a 'Therapeutic Sensory Space' and a 'Sensory Integration Program' designed to assist children with emotional, cognitive, and developmental delays in enhancing their self-regulation skills.

Clarence Central School District Family Support Center, Erie County

The Clarence Central School District Family Support Center has earmarked its grant to provide no-cost direct mental health support for students, addressing social determinants of health such as transportation, financial limitations, and health literacy. As school-based mental healthcare becomes an increasing priority for New York State, the Fidelis Care grant funding will have a direct impact on improving student access and parent education on the importance of mental health and wellness.

Crouse Health Addiction Treatment Services, Onondaga County

Crouse Health Addiction Treatment Services of Syracuse is a leading provider of substance use treatment in Central New York, offering comprehensive inpatient and outpatient programs. Crouse will use the Fidelis Care funding as part of a National Institutes of Health pilot to enhance a substance use disorder facility to improve patient engagement in treatment and improved clinical outcomes.

Exodus Transitional Community, New York County

Exodus Transitional Community of New York City delivers innovative programming tailored to adults and youth affected by the justice system and advocates for a society in which all can achieve social, economic, and spiritual well-being. Exodus plans to use the Fidelis Care grant to increase service delivery to vulnerable populations including young adults, LGBTQIA+, immigrant communities, and individuals impacted by the justice system. Services are trauma-informed, meant to address health equity, and designed to improve both mental and physical health outcomes.

Family Service League, Inc., Suffolk County

Family Service League (FSL), serving children and adults in Nassau and Suffolk Counties, offers more than 60 programs including integrated care, addiction treatment, emergency housing, at-risk youth services, trauma counseling, and senior and caregiver support. FSL plans to use the Fidelis Care grant to provide additional support for its behavioral health crisis hotline (631-952-333), which is part of its critical 24-hour crisis stabilization center and mobile crisis teams.

The HELP USA Fund, Inc., New York County

HELP USA, based in New York City, provides shelter to meet people’s immediate needs, as well as support to succeed as individuals, families, and communities. HELP USA plans to use the Fidelis Care funding to increase social support for formerly homeless individuals who are moving into permanent supported housing programs. 'First Fridays with Fidelis' will be monthly events that highlight support groups for men and women and offer case management as a critical time intervention.

The Huntington Youth Bureau Institute, located in Suffolk County, creates, promotes, and implements programs and services that stress youth development. The Huntington Youth Bureau Institute will use the Fidelis Care funding for bilingual community-based mental health counseling and case management services, available outside of school hours to minimize barriers to access and aid at-risk youth. The Huntington Youth Bureau Institute will also use the funds to equip parents and caregivers with essential mental health resources and enhancing communication skills.

National Education Alliance for Borderline Personality Disorder, New York County

The National Education Alliance for Borderline Personality Disorder provides education, raises awareness and understanding, decreases stigma, and promotes research to enhance the quality of life of those affected by borderline personality disorder. The organization plans to use the Fidelis Care grant to deliver an educational program for parents of youth exhibiting suicidal behavior or ideation. This program is designed to empower both the parent and child with the necessary skills to navigate the challenges faced by a vulnerable, underserved, and marginalized behavioral health population.

Schenectady Family Health Services, DBA Hometown Health Centers, Schenectady County

Schenectady Family Health Services (Hometown Health Centers) helps people live healthier lives by improving health outcomes and health equity through quality primary care and preventive services. It will use the Fidelis Care funding to modernize the electronic health record system of community mental health providers, with the objective of enhancing service delivery. The upgrades will include more sophisticated documentation for mental health treatment goals, comprehensive psychosocial histories, and advanced screening tools to better serve the community's needs.

Yelena Vayntrub Medical PC, Kings County

Yelena Vayntrub Medical PC , a clinic in Brooklyn, plans to use the Fidelis Care grant to establish a comprehensive mental health initiative within a primary care setting aimed at educating patients and fostering community through group activities that enhance interpersonal relationships. This program will extend its reach to engage individuals not currently within the healthcare system and will incorporate Narcan training to address urgent community health needs.
