Fidelis Care is committed to expanding access to behavioral health services and improving mental health and substance use outcomes for individuals and families across New York State. Maintaining mental wellness is just as important as caring for your physical health. Fidelis Care strives to ensure getting high-quality behavioral health care is easy and equitable.

Partnership Spotlight: The Jed Foundation

As part of our commitment to youth mental health, Fidelis Care and the Centene Foundation partnered with The Jed Foundation (JED) to help protect and strengthen the mental health of young people across New York State.

Through this partnership, JED provides consultation, strategic planning, education, and training support to youth-serving community-based organizations, helping equip young people with life skills and connect them to mental health resources when they need them.

Behavioral Health Grant Program  

The Behavioral Health Grant Program provides funding to healthcare providers and community-based organizations across New York State that are working to improve behavioral health outcomes through innovative, integrated approaches.

2025 Behavioral Health Grant Recipients

Center for Comprehensive Health Practice (CCHP) - New York County

  • Who they are: The Center for Comprehensive Health Practice (CCHP) delivers integrated, culturally responsive care to individuals and families living in some of New York City’s most underserved communities.
  • What the grant supports: Expansion of CCHP's adolescent mental health model by incorporating behavioral health and substance use screening into routine wellness visits.

The Desens House - Oswego County

  • Who they are: The Desens House is a community-driven, faith-based nonprofit organization that supports women recovering from substance use disorders through residential programming and stabilization services.
  • What the grant supports: An enhanced Wellness Exploration Program, which integrates accessible fitness, mindfulness, and wellness practices into recovery services.

The Guthrie Clinic - Upstate New York

  • Who they are: The Guthrie Clinic is a nonprofit, integrated healthcare system serving communities across upstate New York.
  • What the grant supports: The strengthening of Guthrie’s collaborative care model within primary care settings, supporting individuals experiencing anxiety and depression.

Horizons Health Services - Erie, Niagara, and Genesee Counties

  • Who they are: Horizon Health Services provides comprehensive, trauma-informed behavioral health treatment and recovery services for children, adolescents, adults, and families.
  • What the grant supports: Expanded community-based outreach and engagement efforts focused on underserved pregnant and postpartum individuals.

Just Kids Diagnostic and Treatment Center - Suffolk County

  • Who they are: Just Kids Diagnostic and Treatment Center offers integrated healthcare services designed to meet the physical, behavioral, and developmental needs of children and families.
  • What the grant supports: Expansion of mental and behavioral health services within a primary care setting, helping reduce stigma and improve access to early evaluation and intervention.

Pediatric Wellness of Northern New York - Jefferson County

  • Who they are: Pediatric Wellness of Northern New York serves children and young adults through medical, behavioral, and educational support programs.
  • What the grant supports: Additional early diagnosis services, behavioral therapy, and parental guidance programs.

Seleni House Foundation - New York County

  • Who they are: The Seleni Institute focuses on improving mental health care for women and families during fertility, pregnancy, postpartum, and early parenting.
  • What the grant supports: Primary care providers training across New York State to better identify and respond to perinatal mental health conditions.

University of Rochester Medicine Connecting for Kids - Monroe County

  • Who they are: The Connecting for Kids (C4K) program enhances collaboration among healthcare providers, schools, and families to better address child and adolescent behavioral health needs.
  • What the grant supports: The C4K Collaborative Dialogue, a community-developed initiative that improves communication and coordination across systems of care.

Westchester Jewish Community Services - Westchester County

  • Who they are: Westchester Jewish Community Services (WJCS) is a comprehensive human services organization supporting individuals and families across the lifespan.
  • What the grant supports: Improved coordination between behavioral health providers and primary care providers within WJCS’ Certified Community Behavioral Health Clinics.
