Fidelis Care is committed to addressing food insecurity as an essential driver of health. Access to nutritious food supports better health outcomes, helps prevent and manage chronic disease, and strengthens families and communities across New York State.

Veggies to Go

Veggies to Go is Fidelis Care’s community-based food distribution initiative that provides fresh, nutritious food to individuals and families across New York State. Events are hosted in partnership with local organizations and often include nutrition education and health information to support healthier food choices. By meeting people where they are, Veggies to Go helps reduce barriers to food access in both urban and rural communities.

Foodsmart

Foodsmart helps eligible Fidelis Care members by providing personal nutrition counseling by phone and custom meal plans to assist with healthy eating. Members also may be eligible to receive up to $125 in Foodsmart Bucks (digital grocery money). Tips on managing weight, diabetes, or other health conditions are also available.

Food Security Grant Program  

In 2025, Fidelis Care awarded food security grants to the following organizations to support innovative programs that integrate nutrition with healthcare, connect families to healthy food options, and address food-related health needs such as chronic disease prevention and management:

Eagle Star Housing – Genesee County

Who they are: Eagle Star Housing supports individuals and families facing housing instability and related health challenges.
What the grant supports: A Food Is Medicine initiative that delivers tailored meals, nutrition assessments, and education to improve chronic disease outcomes and reduce food insecurity.

FeedMore WNY – Erie County

Who they are: FeedMore’s Medically Tailored HDM Program delivers specialty meals to homebound older adults with chronic conditions, supporting independence and reducing healthcare costs.
What the grant supports: A program in collaboration with healthcare providers to conduct nutritional assessments, wellness checks, and social support.

Food Bank of Central New York – Onondaga County

Who they are: The Food Bank of Central New York serves as a regional hub for food distribution and nutrition programming.
What the grant supports: A partnership with Upstate University Hospital to address chronic disease management through access to nutritious food for patients experiencing food insecurity.

Island Harvest – Suffolk County

Who they are: Island Harvest is dedicated to ending hunger on Long Island through food distribution, nutrition education, and supportive services.
What the grant supports: Island Harvest’s Nutrition Pathways Program addresses food insecurity and chronic illness in partnership with hospitals and health centers.

Korean Community Services of Metropolitan New York – Queens County

Who they are: Korean Community Services of Metropolitan New York (KCS) is a nonprofit organization founded in 1973 to serve immigrant and low-income New Yorkers, with a focus on Korean American and broader AAPI communities.
What the grant supports: KCS operates one of New York City’s largest culturally tailored Home-Delivered Meal programs, providing nutritious meals, wellness checks, and case management to homebound older adults in Queens.

Nassau County Department of Health – Nassau County

Who they are: The Nassau County Department of Health has advanced its food security mission from emergency food relief to an integrated public health approach to address chronic disease prevention.
What the grant supports: The Power to Prevent public health education and nutritional education program focuses on chronic disease prevention.

Rural Health Network of South Central New York – Broome County

Who they are: The Rural Health Network of South Central New York works to improve access to healthcare and essential resources in rural communities.
What the grant supports: Expansion of the Produce Prescription Program, which allows healthcare providers to write prescriptions for fresh fruits and vegetables for at-risk patients.

Schoharie County Council of Senior Citizens – Schoharie County

Who they are: The Schoharie County Council of Senior Citizens operates The Gathering Place 50 Plus Community Centers, serving older adults across rural Schoharie, Otsego, and Delaware Counties.
What the grant supports: The Café for a Cause Healthy Lunch and Learn Program provides nutritious meals and  health education for older adults.

United Cultural Association – Kings County

Who they are: United Cultural Association provides community-based services in Brooklyn focused on supporting underserved populations.
What the grant supports: Increasing access to nutritious food and culturally relevant health education for underserved immigrant communities.

United Memorial Medical Center – Genesee County

Who they are: United Memorial Medical Center, part of Rochester Regional Health, is a healthcare provider serving Genesee, Orleans, and Wyoming counties.
What the grant supports: A program that offers guided grocery store tours led by registered dietitians that allows participants to gain practical skills to make healthier, budget-conscious food choices.
