2023-2024 Fidelis Care Maternal Health Grant Program Recipients

In its inaugural round of funding in 2023-2024, Fidelis Care gave $158,000 in maternal health grants to nine not-for-profit groups that focus on helping moms stay healthy. Here’s a recap of how those grants made a difference for 1,320 expectant and new mothers and their babies:


Life of Hope - Kings County

  • Who they are: Life of Hope provides health and wellness services to young people and immigrant families, including support for pregnant and postpartum individuals.
  • What the grant supports: Expanded targeted services for mothers with high-risk medical conditions, helping ensure participants received additional care and follow-up during pregnancy and after birth.

Erie Niagara Area Health Education Center - Erie and Niagara Counties

  • Who they are: Erie Niagara AHEC advances healthcare equity through education and community-based programs.
  • What the grant supports: Connection for expectant mothers—primarily Black and Brown participants—with doulas, education, care navigation, and postpartum support.

Mom Starts Here - Rensselaer County

  • Who they are: Mom Starts Here provides comprehensive support to families throughout pregnancy and early parenting.
  • What the grant supports: Screenings for barriers to prenatal and postpartum care and connections to community health workers for additional support and services for program participants.

MOMMAS House - Nassau County

  • Who they are: MOMMAS House offers residential support for young pregnant and parenting women experiencing homelessness.
  • What the grant supports: Supportive prenatal education, parenting workshops, breastfeeding education, and maternal mental health resources, and connecting participants to behavioral health services as needed.

Healthy Baby Network - Monroe County

  • Who they are: Healthy Baby Network works to ensure that families have the information and resources needed to support healthy pregnancies and infants.
  • What the grant supports: Education, community partnerships, and a large-scale community baby shower that provided essential supplies and health workshops to families.

Oswego County Opportunities - Oswego County

  • Who they are: Oswego County Opportunities supports pregnant individuals and new parents through care management and crisis intervention services.
  • What the grant supports: Strengthening of the OPTIONS program, supporting breastfeeding education, postpartum support, and ongoing care coordination.

Project Stork - Orleans and Genesee Counties

  • Who they are: Project Stork provides maternal mental health support to parents of infants and toddlers.
  • What the grant supports: Screenings, behavioral health referrals, and continued mental health and wellness support during the postpartum period for program participants.

Maternal Infant Services Network (MISN) - Orange, Sullivan, and Ulster Counties

  • Who they are: MISN supports pregnant and parenting families in the Mid-Hudson region through home visiting services, parenting education, and mental health support.
  • What the grant supports: Expanded access to group education, parenting resources, and community baby showers for new families.

North Country Prenatal/Perinatal Council - Jefferson County

  • Who they are: The North Country Prenatal/Perinatal Council works to improve maternal and infant health in rural communities with limited access to healthcare.
  • What the grant supports: Supportive outreach, education, and connection to services for families across Jefferson, Lewis, and St. Lawrence Counties.
