Rural Health Equity

About 60 million Americans live in rural areas , including 1.5 million in New York State. The U.S. Government Accountability Office says people in rural areas face big challenges when it comes to getting healthcare. These include having fewer doctors, needing to travel longer distances to reach medical care, and not having many specialized services nearby. Many rural areas also lack good internet, making it harder to use telehealth, which helps people get care online.

Fidelis Care gave $128,000 in rural health grants to nine local organizations in New York. These groups focus on helping people in rural communities, especially those with low incomes and high needs.


Fidelis Care Rural Health Grant Recipients

AIM Independent Living Center - Steuben County

AIM provides a wide range of services to people with disabilities and others with unmet needs.  AIM’s Community Nutrition Program offers a food pantry in Elmira, open five days a week for walk-ins and providing free delivery to residents of Chemung, Steuben, and Schuyler counties who face disabilities and other transportation barriers. The food pantry serves nearly 100 households per month with the delivery service serving more than 400. AIM will use its Fidelis Care grant to add more cold storage space, with the goal of purchasing six new commercial refrigerators and freezers to increase the number of people served by ten percent.

Cornell Cooperative Extension - Broome County

Cornell Cooperative Extension of Broome County puts knowledge to work in pursuit of economic vitality, ecological sustainability, and social well-being. The grant supports its SNAP-Ed (Supplemental Nutrition Assistance Education Program) Fruit and Vegetable Prescription initiative. SNAP-Ed will provide fruit and vegetable coupons to 75 individuals identified by local health care professionals to be used at Broome County Farmers Markets. Individuals will receive their coupons in $20 increments for every SNAP-Ed nutrition education program they attend, up to six programs.


Cornell Cooperative Extension - Steuben County

Cooperative Extension of Steuben County improves lives and communities through partnerships that put experience and research knowledge to work. Extension staff and trained volunteers deliver education programs on topics such as commercial and consumer agriculture, nutrition and health, youth and families, finances, energy efficiency, economic and community development, and sustainable natural resources. The grant expands the current Veggie Van program, providing fresh produce to vulnerable residents in rural areas.

Dutchess Outreach - Dutchess County

Dutchess Outreach is the largest food security organization in Dutchess County, helping 10,000 residents annually. The organization operates through a holistic approach including a food pantry, The Lunch Box community meal program, and Farm Stand free monthly food distribution. Dutchess Outreach is using its Fidelis Care grant in partnership with Sun River Health to operate a mobile food pantry that serves 300 households a year in the town of Armenia.

Fort Drum Regional Health Planning Organization - Jefferson County

Fort Drum Regional Health Planning Organization (FDRHPO) works to improve and strengthen the health and wellness of all individuals living in the North Country by building a patient-centered system of health. FDRHPO's network includes local hospitals, primary care offices, behavioral healthcare providers, EMS agencies, and a variety of community-based organizations. The grant will support a pilot program aimed at improving pediatric oral health through collaboration with local primary care practices. It will implement fluoride varnish treatments as part of routine visits to address a lack of regular dental care and the shortage of dental providers in the region.


Genesee Orleans Ministry of Concern - Orleans County

Genesee Orleans Ministry of Concern (GOMOC) helps people in need in the communities it serves with personal care items, prescription co-pays, vision and dental assistance, certain utility bills, and limited emergency shelter assistance. Youth development programs also provide young people with the skills they need to succeed. GOMOC plans to use its grant to supply hygiene and non-perishable food closets in school districts in Genesee and Orleans Counties.

Literacy West NY, Inc. - Allegany County

Literacy West NY, Inc. (LWNY) is committed to serving diverse and rural populations by offering workshops, classroom-based instruction, and community outreach that focus on practical health information. The Fidelis Care grant will help LWNY launch a health literacy program to empower adults with knowledge and skills for healthier living. Participants will gain tools to navigate healthcare systems, understand preventive care options, and make informed health-related decisions for themselves and their families.

Norfolk Raymondville Food Pantry - St. Lawrence County

The Norfolk Raymondville Food Pantry (NRFP) serves families in the Norwood, Norfolk, and Raymondville area by providing them with nutritious food items through monthly distributions and emergency call response.  NRFP also offers personal hygiene and baby items such as soap, shampoo, diapers, and wipes. The Norfolk Raymondville Food Pantry plans to use the Fidelis Care grant to source food and supplies for its clients through the Food Bank of Central New York.

Restoration Society, Inc. - Genesee County

Restoration Society, Inc. (RSI) is a peer-led organization that has served Western New York for over 50 years, focusing on mental health recovery. Its Genesee ACE Employment program extends personalized support to residents in rural counties like Genesee and Orleans, where barriers to healthcare, employment, and community participation are especially pronounced.  RSI plans to use the Fidelis Care grant to offer digital skills training to help participants connect with telehealth services and navigate essential healthcare resources, along with guidance on community transportation options to ensure they can access jobs, medical appointments, and other critical services.

Access to behavioral healthcare and wellness programs ensures healthy individuals. Fidelis Care is dedicated to helping improve access to behavioral health services and outcomes.


 

Fighting Food Insecurity

Fidelis Care is committed to fighting food insecurity. Access to nutritious foods is a key way to improve health. We have several programs to help get healthy food to people in need.

Fidelis Care offers special programs and works with providers and community partners to address maternal health issues through education, outreach, and community events.
