Get paid for taking care of your health. Starting January 1, 2026, Ambetter from Fidelis Care members are eligible for incentives for completing eligible health activities and screenings.

Sign up starting on January 1, 2026
  1. Register: Visit URL to set up your account.
  2. Log in to view all eligible health activities and potential rewards.
  3. Attest your participation by completing a quick form.
  4. Receive a debit card after you complete your first activity. Funds will be added to your debit card after you complete additional eligible activities.
  5. Shop online for eligible items with your card.
  6. Don’t forget! Unused dollars will roll over to the following month, however, your dollars expire on January 31 of the following year.

Eligible Ambetter Wellness Rewards activities

Effective January 1, 2026

Seeing your primary care provider every year helps keep you healthy, and you can earn a reward for completing your annual Adult Well Visit. At your visit, talk to your PCP about whether you qualify for other Wellness Rewards activities.

Activity Description
Adult Well Visit: $25 Get your free yearly checkup with your primary care provider. This visit helps you stay on top of your health, manage any ongoing conditions, and catch potential issues early.*
Adult Immunization – Flu Shot: $25 An annual flu shot helps protect you and those around you from the flu. It’s quick, safe, and can prevent serious illness. Talk to your primary care provider if you have any questions.
Asthma Medication Ratio/Management: $25 Take your asthma medications as prescribed by your provider. Regular use of your controller medication helps keep your breathing under control and reduce flare-ups.
Breast Cancer Screening: $25 Complete your mammogram to screen for breast cancer. Early detection can lead to a better outcome.
Cervical Cancer Screening: $25 Get your Pap test or HPV screening as recommended by your provider. This simple test helps detect changes early, when they’re easiest to treat.
Childhood Immunizations: $25 Keep your child’s vaccines up to date. Immunizations protect your child from serious diseases and help them stay healthy.**
Blood Pressure Screening: $25 Have your provider check your blood pressure. Keeping it in a healthy range protects your heart, kidneys, and overall health.
Colorectal Cancer Screening: $25 Complete your colorectal screening (FIT kit, colonoscopy, or other test) as recommended. This important test can help catch cancer early.
Diabetes – HbA1c Screening: $25 Get your A1c test at least once a year. It measures your average blood sugar over time and helps you and your provider manage your diabetes effectively.
Diabetes – Eye Exam: $25 Get your yearly diabetic eye exam to check for early signs of eye disease. Regular exams can help protect your vision.
Diabetes – Statin Therapy: $25 Take your prescribed statin medication to help manage cholesterol and reduce heart disease risk.
Kidney Health Evaluation: $25 Get your annual kidney health tests (blood and urine). These screenings help detect early kidney changes caused by diabetes or high blood pressure.
Statin Therapy – Cardiovascular Disease: $25 Take your prescribed statin as directed to help lower cholesterol and reduce your risk of heart attack and stroke. Staying consistent with your medication is key to protecting your heart health.


*Some diagnostic tests or screenings ordered during your annual checkup may be subject to cost sharing. Refer to your plan documents and ask your doctor about possible costs.

**Vaccines listed below must be completed by the child’s second birthday. One reward can be received per child in the household once all vaccines are completed.

  • Diphtheria, tetanus and acellular pertussis (DTaP);- 4 doses
  • Polio (IPV)- 3 doses
  • Measles, Mumps and Rubella (MMR);- One dose
  • Haemophilus influenza type B (HiB);- 3 doses
  • Hepatitis B (HepB),- 3 doses
  • Chickenpox (VZV);- One dose
  • Pneumococcal conjugate (PCV); 4 doses
  • Hepatitis A (HepA); One dose
  • Rotavirus (RV); 2 or 3 doses
  • Influenza (flu)- 2 doses
