To expedite the processing of Urgent Requests, providers should submit requests through:

• Availity Essentials, or
• Fax the request labeled as “Urgent” directly to:
  • Physical Health - (800) 860-8720
  • MLTC – (833) 710-1772
  • Behavioral Health –
    • Mental Health - (833) 561-0094
    • Substance Use Disorder – (833) 663-1608

Urgent Concurrent Authorization Requests and Concurrent Authorization Requests should be submitted by:

• Faxing the request labeled as “Concurrent” directly to:
  • Physical Health - (800) 860-8720
  • MLTC – (833) 710-1772
  • Behavioral Health –
    • Mental Health - (833) 561-0094
    • Substance Use Disorder – (833) 663-1608
