Mail Order Pharmacy Program


One of the best ways to take care of your health is to take your medications as prescribed. In support of this goal, Fidelis Care offers the Mail Order Pharmacy Program to our Medicare Advantage and Dual Advantage members. 

The Mail Order Pharmacy Program, managed by Fidelis Care's pharmacy partner, CVS Caremark (by clicking this link, you will leave the Fidelis Care website), delivers 90-day supplies of your medications to your home at no extra cost to you, without the need for reminder phone calls or trips to the pharmacy. 

Mail Order Pharmacy Program Benefits

  • Cost Savings. One 90-day supply of your medications may cost less than three 30-day supplies at a retail pharmacy.
  • Greater Convenience. The program offers home delivery, at no extra cost. 
  • Automatic refills. Medicines marked for automatic refill will always arrive a few days before you run out. 
  • Quality and safety. Registered pharmacists check each order. 
  • Added value. Receive services such as alert messages by email, text, or phone when you register online with CVS Caremark (by clicking this link, you will leave the Fidelis Care website.)
Eligible Medications

All medications within tiers 1 through 4 of Fidelis Care's Medicare Drug Formulary are eligible for 90-day supply prescriptions. If you need help finding out if your prescriptions are covered in the Fidelis Care Drug Formulary, please call Member Services at 1-800-247-1447 (TTY: 711).

How to Enroll

Via U.S. Mail: 

  • Fill out the Mail Order Enrollment Form (PDF)
  • Mail the form to Fidelis Care's pharmacy partner, CVS Caremark:

For a complete listing of plans in your service area, contact the plan.  Fidelis Care is contracted with Medicare for HMO, HMO D-SNP, and HMO-POS plans, and with the state Medicaid program. Enrollment in Fidelis Care depends on contract renewal.

From October 1 to March 31, you can call us 7 days a week from 8 a.m. to 8 p.m. From April 1 to September 30, you can call us Monday through Friday from 8 a.m. to 8 p.m. A messaging system is used after hours, weekends and on federal holidays.
