NOW ACCEPTING APPLICATIONS
Fidelis Care is awarding Here for Your Health maternal health grants to healthcare providers and community-based organizations in New York State to help support programs that advance innovative solutions to improve postpartum visit adherence and expand postpartum depression screening.
If your office or organization runs a program that helps supports these types of initiatives, particularly for high-need, low-income women and their families, then Fidelis Care encourages you to apply for the grant.
Grants up to $20,000 will be awarded. To be eligible, providers or community organizations must serve New York State.
Applications will be accepted until 5 PM on September 21, 2026. Applicants must must operate a program focused on postpartum visit adherence and/or postpartum depression screening and answer all of the questions on the form to be considered.
Applications are no
longer being accepted at this time. Organizations will be contacted in October about their requests.
