For providers who participated in the 2018 Medicaid Chart Review, you can find the supplemental diagnosis codes that Fidelis Care submitted to the New York State Department of Health by logging in to Provider Access Online (provider portal). Select the ‘File Downloads’ menu option and search for the document name that begins with “Medicaid Risk Adjustment – Supplemental Diagnosis Codes”, and then click Download. If you do not already have access to Fidelis Care’s provider portal, please contact the Provider Call Center at 1-888-FIDELIS (1-888-343-3547).
