Submission Deadline Approaching: Medicaid Charts for Review

If you have not yet done so, please submit your Fidelis Care Medicaid charts for review. Fidelis Care is required by the New York State Department of Health to submit complete diagnostic data for Medicaid members for Risk Adjustment. To help us obtain the necessary medical records for submission, we have partnered with Optum and CiOX Health.
If you have received a list of Fidelis Care Medicaid members whose records we need for service dates starting January 1, 2019 to December 31, 2019, we kindly ask that you provide these records by July 30, 2020. If you do not have a list of these members, please contact the number below.
Records can be submitted by fax, mail or via the CiOX Portal.
