Effective December 1, 2020, Fidelis Care is partnering with Envolve Pharmacy Solutions to implement a Polypharmacy program in which members utilizing 8 or more medications will be reviewed and identified for drug-related problems such as therapeutic duplications, drug-drug/drug-age interactions, fraud and waste, and abuse or use of medications for longer than the recommended duration. Providers will receive a letter, followed by a phone call from a licensed pharmacist, to discuss recommended changes.
The Polypharmacy program provides an opportunity to positively impact member health outcomes.
