Authorization Changes Effective October 15, 2021 – DME, GI, and Sleep Study (Updated)

Effective October 15, 2021, Fidelis Care will be moving a number of authorization requests to our portal and our fax line. It is important to note these services will no longer be authorized via phone unless considered an urgent clinical request. 

Services related to DME:

  • DME authorizations that were previously approved immediately over the phone for Metal Level Products, Managed Medicaid, CHP, and Medicare Advantage/Flex – must be submitted to our fax line at 716-200-1389
  • DME authorization requiring clinical review for Metal Level Products, Managed Medicaid, CHP, and Medicare Advantage/Flex & Out of Network Providers – must be submitted to our fax line at 1-800-860-8720.  
  • All requests for MLTC DME authorization – must be submitted to our fax line at 716-803-8727

Services related to GI

  • GI authorization for Out of Network providers - must be submitted to our fax line at 1-800-860-8720
  • GI authorization for Metal Level Products, Managed Medicaid, CHP, and Medicare Advantage/Flex – must be submitted to our fax line at 716-200-1389

Services related to Sleep Studies

  • Sleep Studies performed in home - must be submitted to our fax line at 716-200-1389
  • Sleep Studies performed in any other place of service  & Out of Network Providers– must be submitted to our fax line at 1-800-860-8720

Log in to Provider Access Online (The Provider Portal) at your own convenience. The system can be accessed at any time, 24 hours a day, 7 days a week.
