MOST VULNERABLE PATIENTS (MVP) Incentive - A subset of the Continuity of Care (CoC) Program

Fidelis Care has identified its most vulnerable patients (MVPs) in the Medicare Continuity of Care (CoC) program. We would like to partner with you to help care for these patients.

To offset any extra administrative costs for scheduling these members between November 1, 2021 and December 31, 2021, we are offering $300 for each completed, verified, and payable Appointment Agenda received between November 1, 2021 and January 31, 2022, for any Date of Service between November 1, 2021 and December 31, 2021.

Submitted Appointment Agendas with active diagnoses must be verified on the claim.

WHAT YOU NEED TO DO!

  • Reach out to your patients to schedule appointments before December 31, 2021
  • Use the Appointment Agenda as a guide
  • Update diagnoses and close care gaps, and document these in the medical record and on the claim
  • Sign, date, and submit the signed Appointment Agenda via fax (1‐813‐464‐8879), email (agenda@centene.com), or the secure Provider Portal
  • Submit a claim/encounter containing all relevant diagnosis codes
