HealthierLife is Fidelis Care's Health and Recovery Plan (HARP), a new kind of plan that provides people who are eligible for Medicaid and who have specific behavioral health needs with an enhanced benefit package so that they can receive all of their physical health, behavioral health, and other supportive services from one integrated plan. HealthierLife members are also eligible to become a member of a Health Home, which provides members with their own Health Home Care Manager to assist with coordinating care across all of their providers.
HealthierLife is designed to promote a recovery-oriented model of care-- one that prioritizes and supports a member's potential for recovery by focusing on quality of life and reducing barriers caused by their mental illness and/or substance use disorders through empowerment, choice, treatment, education, vocation, housing, health, and well-being goals. To accomplish these goals, Fidelis Care partners with Health Homes to offer a team-based approach to care coordination that addresses a member's physical, behavioral, and social needs.
As a member of HealthierLife, you may qualify for extra benefits and supports to help you achieve the best possible results from your care, including help preparing for and finding a job, finding housing, going to school, or reaching other life goals. These new benefits are called Home and Community Based Services (HCBS). Before you can receive these services, you will need to participate in an assessment to determine which services you are eligible to receive. Your Health Home Care Manager will be able to perform this assessment and help you determine which services will best meet your individual needs. If you don't belong to a Health Home, Fidelis Care will identify someone who can perform this assessment for you to ensure you can get the services you need.
Who can join HealthierLife?
At this time, only Medicaid-enrolled individuals age 21 or older who have been identified by New York State as eligible to join a Health and Recovery Plan can enroll in HealthierLife.

A drug formulary is a list of both generic and brand name prescription drugs that are covered by your prescription drug benefit. The formulary is managed by our Pharmacy and Therapeutics Committee, qualified pharmacists and doctors whose primary focus is offering safe, high-quality and cost-effective drugs. New drugs are added to the formulary periodically based on the latest medical research and some drugs may be removed periodically based on the findings and recommendations of the Pharmacy and Therapeutics Committee.
