HealthierLife (HARP)

HealthierLife is Fidelis Care's Health and Recovery Plan (HARP) for qualifying Medicaid Managed Care members. This plan offers an enhanced benefit package for people who have specific behavioral health needs.

Who can apply for HARP? 

At this time, only Medicaid-enrolled individuals age 21 or older who have been identified by New York State as eligible to join a Health and Recovery Plan can enroll in HealthierLife.  

How to enroll

Enrollment is automatic if you are enrolled in New York State Medicaid and have been identified by New York State as eligible to join a Health and Recovery Plan.

Benefits

HealthierLife promotes recovery for members by helping to remove quality-of-life roadblocks caused by behavioral health issues and/or substance abuse. Members receive all physical, behavioral, and social health and support services, including:

  • No-cost annual checkups
  • Immunizations and preventive care
  • Hospital and emergency care
  • Transportation to doctor visits
  • Eye and dental care
  • Adult mental healthcare, including psychiatric services
  • Adult outpatient mental healthcare, including continuing day treatment
  • Behavioral Health Home and Community Based Services (BH HCBS)*
  • Substance use disorder services, including detoxification and support

    Home and Community Based Services*

    As a HealthierLife member, you may qualify for extra benefits known as Home and Community Based Services (HCBS). These benefits offer individualized supports, including:

    • Help preparing for and finding a job
    • Finding housing
    • Going to school
    • Reaching your life goals
    Members participate in an assessment to determine which services they are eligible to receive. 

    Through Fidelis Care's partnership with Health Homes, members are also eligible for their own Health Home Care Manager and Fidelis Care Care Manager to assist with coordinating care across all of their health care providers.
