Prescription Discount Program

As a member of Ambetter from Fidelis Care, you may have access to an easy, no-cost program that can give you discounts on certain medications. Here’s what you need to know about the prescription discount program offered through Sempre Health and how to get started:


What is Sempre Health?

Sempre Health helps eligible members save money on a variety of prescription medications, including certain medications for cardiovascular, respiratory, and diabetic conditions.

How do I know if I am eligible for this program and if my medications are eligible?

You would have received a letter or email inviting you (or a member of your household) to the Sempre Health program letting you know of an eligible prescription. If you have any questions, please call 1-855-913-5193 to speak with Sempre Health’s live member support team Monday – Friday between 9 a.m. and 8 p.m. Eastern Time/Eastern Standard Time/EST.

How do I use Sempre Health?

Step 1. You’ll get an email or letter invitation from Ambetter from Fidelis Care and Sempre Health inviting you to sign up.

Step 2. In that email or letter invitation, you’ll see a phone number and unique code. Text that code to the provided phone number, and Sempre Health will text you back a discount ID. Or, you can use your unique code to sign up on the Sempre Health website.

Step 3. Show the pharmacist your Sempre Health discount ID to activate your savings.

Step 4. Sempre Health will text a reminder to your phone whenever it’s time to refill your prescription, and your discounts will be applied at the pharmacy automatically.


By enrolling in Sempre Health, you authorize Sempre Health to send you up to four texts per week using an autodialer to the number you provided. Consent is not a condition of purchase. Your enrollment is your electronic signature agreeing to these terms and to giving electronic written consent. Reply “HELP” for help; reply “STOP” to withdraw consent. Message and data rates may apply.

Ambetter Health has contracted with Sempre Health, an independent company, to provide select discounted prescription copays to eligible Ambetter Health members. Savings will vary based on drug, member copayment, and program requirements. You will never pay more than your typical copay. Maximum discount limits apply. This discount program is not insurance.

Important Coverage Changes

Your monthly insurance costs could increase due to changing government support.

The tax credits that help lower monthly premiums will be reduced in 2026. This year more than ever, it’s important to update your enrollment application and income to ensure you get the most financial assistance available to you.

Types of Income Verification Documents

Document types accepted by the government include:

  • 1040 Federal or State Tax Return
  • Wages and Tax Statement
  • Pay Stub
  • Self-Employment Ledger Documentation
  • Social Security Administration Statements
  • Unemployment Benefits Letter
  • “Written Explanation Form”

Renew your coverage during Open Enrollment.

Renew by December 15, 2025, for January 1, 2026, coverage. If you miss the December 15 deadline, you can still enroll for 2026 coverage by January 31, 2026, but you will have a lapse in coverage. Open Enrollment may be the only time you can enroll unless you have a qualifying life event.

How To Renew

Take time to renew – your health is worth it.

Before it's time to renew, you will receive a notice by mail, email, or phone that will tell you everything you need to know about renewing your coverage. If you have questions about renewing your current coverage or your healthcare needs have changed: 

During the annual Open Enrollment period, you can also renew your coverage directly with the NY State of Health. To get started:

  1. Visit www.nystateofhealth.ny.gov and click on "Get Started."
  2. Follow the instructions to create a ny.gov ID if you don't already have one.
  3. After logging in, you will need to enter your invitation code. This is included in the letter you received from NY State of Health (located under step 2 in the instructions).
  4. Follow the instructions on the NY State of Health website to finish your renewal.
