Formulary Changes – Fourth Quarter 2022

Fidelis Care has made changes to our Medicaid Managed Care/HealthierLife (HARP) and Child Health Plus (CHP) Formularies and Qualified Health Plans (Metal Level Products) and Essential Plan (EP) Formularies. Some of the changes relate to quantity limitations, authorization requirements, as well as drugs being removed and added to the formulary.

 

Please refer to the Medicaid Managed Care/HealthierLife (HARP) OTC Removals, updated in accordance with the New York State Department of Health requirements, and the Medicaid Managed Care/HealthierLife (HARP)/Child Health Plus (CHP) OTC Additions.
