Advance Directives

Advance directives are legal documents that ensure your doctors know the care you want in the event you cannot make decisions for yourself. These documents provide instructions for the care you wish to be given under certain circumstances. They can also name a family member or friend to make decisions on your behalf.

Advance directives include:

Healthcare proxy

A healthcare proxy is someone designated to make healthcare decisions if you cannot. The form must be signed and witnessed by two adults.

Living will

A living will is a written statement about your wishes for care. It does not appoint a decision-maker, or healthcare proxy.

Non-Hospital Do Not Resuscitate order

A DNR is a medical order to not give CPR or other emergency measures if your heart or breathing stops.

Many people also have a licensed healthcare provider sign Medical Orders for Life-Sustaining Treatment. These orders can be used as soon as they are signed. MOLSTs include a list of treatments you do and do not want, such as CPR, a breathing tube, or a feeding tube.

You have the right to create advance directives according to your own wishes.

It is important for you to document how you would like your care to continue if you are no longer able to communicate with healthcare providers.

If you have a care manager, they can help you complete these forms. If you already have an advance directive, please share a copy with your care manager.
