Here’s how to get started:

Step 1: Sign Up for Our Member Portal

The Member Portal is the easiest and most secure way to access information about your plan. With 24/7 access to your Fidelis Care account, you can pay your monthly premium, request and print new ID cards, and access your plan documents. To create a secure online member account, visit members.fideliscare.org and click “Register.”

Step 2: Fill Out Your Health Risk Assessment

You can complete this brief survey on the Member Portal. The answers you provide will help us serve you better and get you connected to whatever services you may need.

Step 3: Review Your Plan Documents

You can access your Summary of Benefits and Coverage,Subscriber Contract, Formulary, and other important plan documents by logging into the Member Portal. The Summary of Benefits and Coverage outlines some of your plan’s key benefits, and explains how much they could cost. The cost examples are estimates only, and serve as a guide for how your particular plan might cover different conditions; not the exact dollar amount you will pay. Visit fideliscare.org/costcalculator to find a treatment cost calculator, providing you with estimates that cover other conditions and services. Your Subscriber Contract provides a full description of the services included in your benefit package, as well as cost information for those services. 

Step 4: Choose a Primary Care Provider
Fidelis Care is committed to ensuring you have access to the preventive care and screenings you need to stay healthy. To ensure that access, we recommend you select a Primary Care Provider (PCP), even if your plan does not require one. If you already have a PCP who is in the Fidelis Care network, then that provider should have been automatically assigned as your PCP. You can review, select, or change your PCP from your Member Portal account. Your PCP will work with you to make sure you get the care you need.

Pharmacy Benefits

For members with pharmacy coverage, your benefits are covered through CVS Caremark. Visit the CVS Caremark website (by clicking on this link, you will leave the Fidelis Care website) for more information or to register.

Dental Benefits

For members with dental coverage, your benefits are covered through DentaQuest. Visit our DentaQuest Resource Page (by clicking on this link, you will leave the Fidelis Care website) for more information on getting started, and preventive dental care best practices.

Vision Benefits

Member Payment Portal Update

Fidelis Care's online payment portal will be upgraded on September 17, 2022. The redesigned payment system includes a new, easy-to-use interface and more features to improve your experience. Paying online is a quick and easy way to make a SECURE payment for your monthly Fidelis Care premium. This service is available 24 hours a day, 7 days a week, 365 days a year.

Information on the Polio State Disaster Emergency

In response to the polio state disaster emergency, Fidelis Care urges our unvaccinated members to get immunized to protect yourself from this serious disease.

All New Yorkers who are unvaccinated, including children 2 months and older, those who are pregnant, and people who have not completed their polio vaccine series, should get immunized.

Polio is a highly contagious disease that can cause paralysis. Poliovirus, the virus that causes polio, spreads from person to person through the mouth or nose. It can get into food and water.

The first case of polio in the United States since 1994 was recently detected in Rockland County. Poliovirus was also found in wastewater samples in the greater New York City area. If you are unvaccinated and live, work, visit, or attend school in Rockland County, Orange County, New York City, Sullivan County, and Nassau County, you are at the highest risk of contracting the disease.

Vaccination is the best way to protect yourself from polio. Inactivated poliovirus vaccine (IPV) is safe and does not contain live virus. People who get the vaccine cannot infect others. IPV is the only polio vaccine used in the United States.

IPV vaccination is part of a child's recommended routine vaccinations. More than 99 percent of children who get all four IPV doses (at 2 months old, 4 months old, 6 through 18 months old, and 4 through 6 years old) will be protected from polio. Unvaccinated adults should receive 3 doses.

If you are not sure whether you and your child have received the complete polio vaccine series, contact your provider to see if they have your immunizations records. If your provider does not have the records and you are not sure of your vaccination status, then it is recommended that you get vaccinated.

Contact your provider to schedule your polio vaccination. If you live in Rockland County, you also can visit a polio vaccine clinic.

The New York State Department of Health also recommends that the following New Yorkers who have previously completed their polio vaccine series should receive one lifetime booster dose of IPV:

  • Individuals who may have close contact with a person or family member known or suspected to be infected with poliovirus.
  • Healthcare workers working in areas where poliovirus has been detected who may be exposed to poliovirus.
  • People who work in the wastewater industry.

It is also important to wash hands often with soap and water. Alcohol-based hand sanitizers do not kill poliovirus.

Fidelis Care does not reward providers or other individuals for issuing denials of coverage, and does not offer financial incentives to UM staff based on decisions that promote underutilization of services.
