Living Well with Diabetes: Your Guide to Better Health
11/5/2025 • Posted by Dr. Natalie Schwartz, Endocrinologist in Diabetes, Health and Wellness

Diabetes-Awareness-Month

Managing diabetes can feel challenging — but you don’t have to do it alone. With the right care, daily habits, and support, you can live a healthy and active life. At Fidelis Care, we’re here to help you take meaningful steps toward better health.

What Is Diabetes?
Diabetes affects how your body turns food into energy. If your body doesn’t make enough insulin, or can’t use it effectively, blood sugar levels can rise — increasing the risk for serious health issues over time.

The good news? With good medical care and healthy lifestyle choices, you can manage your diabetes and feel your best.

Small Steps Make a Big Difference
Eat Well

  • Choose whole grains, vegetables, lean proteins, and fiber-rich foods
  • Limit sugary drinks and processed snacks
  • Talk to your provider about a meal plan that works for you

Stay Active

  • Aim for at least 150 minutes of physical activity per week
  • Walking, stretching, or light exercise can help lower blood sugar
  • Always check with your doctor before starting a new routine

Know Your Numbers

  • Monitor your blood sugar as directed by your provider
  • You may be eligible for a continuous glucose monitor (CGM), which tracks your blood sugar in real time. Ask your provider if it’s right for you and if it’s covered under your plan
  • Ask about A1C testing to check your average blood sugar over time
  • Track your blood pressure and cholesterol — they matter, too

Care for Your Mind and Body

  • Stress and lack of sleep can impact your blood sugar
  • Practice relaxation techniques like deep breathing or gentle yoga
  • Reach out for mental health support if you’re feeling overwhelmed

What’s New in Diabetes Care
Diabetes care continues to evolve — and more tools are available to help you stay healthy and in control.

Innovative Medications

Newer medications are available that not only lower blood sugar, but may also:

  • Support healthy weight loss
  • Protect the heart, blood vessels, and kidneys
  • Lower the risk of serious diabetes-related complications

Talk to your provider about whether these medications are a good option for you.

Easier Access to Continuous Glucose Monitoring (CGM)

Many health plans now offer expanded coverage for CGMs, which make it easier to:

  • Track blood sugar trends
  • Avoid extreme highs and lows
  • Understand how food, activity, and stress affect your body

Check with your provider and Fidelis Care to see if you qualify for a CGM.

Diabetes Self-Management Education and Support (DSMES)

Managing diabetes is about more than medications. DSMES programs can teach you how to:

  • Build healthy habits
  • Read labels and plan meals
  • Set goals and manage stress
  • Stay motivated and confident in your care

Programs may be available through your provider, local health clinics, or virtually.

We’re Here for You
Fidelis Care can work with your doctor or primary care provider to support your diabetes journey with:

  • Care coordination and support for managing chronic conditions like diabetes
  • Preventive health screenings to catch issues early and stay on track
  • Coverage for diabetes medications and supplies
  • Nutritional counseling and health education resources
  • Help scheduling follow-up care and appointments

Provider Office Hours

Fidelis Care hosts webinars called “Provider Office Hours” several times each month to share information and answer any of your questions.

Provider Satisfaction Survey

Your provision of quality care and services to our members contributes greatly to Fidelis Care’s success. One of the ways Fidelis Care prioritizes quality is making sure you are satisfied with your experiences. Our annual Provider Satisfaction Survey identifies areas to focus on for further improvement in the coming year. The Provider Relations Department works diligently to identify an action plan to address areas of improvement, while continuing to develop actions and interventions to improve the provider experience.

HEDIS® Measurement Toolkit

HEDIS® is a tool used by more than 90% of America’s health plans to measure performance on important dimensions of care and service. We've developed this toolkit to help identify required preventive services.
