In an effort to streamline provider termination submissions to Fidelis Care, we have created a new electronic Termination Request Form for providers to use. Please use this form to submit any requests to terminate a provider from a TIN.

Wellcare Fidelis Dual Plus is a health plan for individuals who have Medicare and Medicaid coverage. Enrollment in the program includes Medicare, Medicaid and long-term-care benefits, allowing for better coordination of care for our members.
