Fidelis Care works to positively impact the lives of New Yorkers through access to quality health coverage and care. As part of that mission, we provide health resources and information to help people live healthier lives.

Here to Help

Care Management

Care Managers help members who need extra health support. Your Care Manager can help coordinate your care, learn more about your health, or help you find helpful resources in your community.

Many members qualify for case management, including those with health conditions or in situations such as:

  • Asthma
  • Breast Cancer
  • Cancer
  • Controlling high blood pressure and living with a severe and persistent mental illness such as schizophrenia, bipolar disorder, and major depression
  • Diabetes
  • Chronic obstructive pulmonary disorder (COPD)
  • Congestive heart failure (CHF)
  • After a stroke
  • After a hospitalization or emergency room visit
  • After you didn’t receive the care you needed
  • Having multiple diseases happening at once
  • Needing a life-sustaining device, such as a left-ventricular assist device (LVAD)
  • After a recent decline in function within 90 days
  • Having a high-risk pregnancy (including pregnant while taking 17-P)
  • Being an organ transplant candidate
  • Needing newborn intensive care
  • Catastrophic illness or injury
  • Sickle cell disease
  • Hemophilia
  • HIV/ AIDS
Population Health Management

Help is also available from our Population Health Management programs. They can help you by:

  • Reminding you to have your flu shot or other care you may need
  • Referring you to Health Homes 
  • Offering services for behavioral and physical health needs, like those listed above
  • Helping ensure your medications work together
  • Coaching you to support self-care habits
  • Controlling high blood pressure and living with a severe and persistent mental illness such as schizophrenia, bipolar disorder, and major depression
